Abstract

Background. Chronic pyelonephritis remains a significant problem in nephrology and general pathology, particularly among women of reproductive age. The high prevalence of the disease, its recurrent course, and associated complications negatively affect reproductive health and life expectancy. Chronic pyelonephritis is considered a factor of increased cardiovascular risk, which makes the study of central hemodynamic parameters particularly relevant.


Objective. To investigate the characteristics of central hemodynamic parameters in women of reproductive age with chronic pyelonephritis.


Materials and Methods. A total of 34 women aged 17–39 years with chronic pyelonephritis were examined. All patients underwent clinical and laboratory assessment, functional and instrumental evaluation of renal status, as well as echocardiography with Doppler imaging. Systolic and diastolic left ventricular function, cardiac chamber dimensions, and left ventricular myocardial mass index were analyzed.


Results. A recurrent form of the disease was diagnosed in 56.0% of patients, anemic form in 15.0%, hypertensive form in 15.0%, and septic form in 9.0%. No significant impairment of systolic function according to ejection fraction was observed. Left ventricular hypertrophy was detected in 55.8% of patients, with an increased left ventricular mass index. Diastolic dysfunction of type I (impaired relaxation) was identified in 44.1% of cases.


Conclusion. Women of reproductive age with chronic pyelonephritis demonstrate a significantly higher prevalence of left ventricular hypertrophy and diastolic dysfunction, indicating a close relationship between renal dysfunction and cardiovascular system alterations.
