Endovascular mechanical thrombectomy has revolutionized the treatment of acute ischemic stroke, extending the therapeutic window for intervention up to 24 hours in carefully selected patients. Recent multicenter randomized trials such as DAWN and DEFUSE-3 demonstrated that clinical condition and neuroimaging criteria—specifically, the infarct core volume (<70 mL) and ischemic tissue-to-core ratio (≥1.8)—are more critical than the time from symptom onset. The use of perfusion-based imaging processed with RAPID software has enabled precise identification of patients who benefit from revascularization. Endovascular treatment significantly improves functional outcomes and reduces mortality compared with standard therapy, confirming its efficacy and safety even beyond the traditional 6-hour limit.
