RESULTS OF SURGICAL STRATEGY FOR FEMORAL NECK FRACTURES IN ELDERLY PATIENTS

Abstract
The aim of this study was to optimize the surgical strategy for treating femoral neck fractures in elderly patients based on Doppler ultrasonographic assessment of vascularization in the proximal femur. The research included patients treated at the Department of Traumatology and Orthopedics, Samarkand State Medical University, between 2020 and 2025. Doppler ultrasonography enabled differentiation of three vascularization types (compensated, subcompensated, and decompensated), allowing for individualized selection of the surgical approach—osteosynthesis or total hip arthroplasty. The developed technique is noninvasive, simple to perform, and highly informative. Integration of Doppler-based perfusion assessment into preoperative planning significantly reduced postoperative complications and improved functional recovery in elderly patients with femoral neck fractures.
