Abstract
Aim — to define the ultrasound (US) morphological types of gallbladder cancer and to study the agreement between US assessment of wall invasion depth and the histopathological T category. Material and methods. Thirty-four patients with histologically confirmed gallbladder cancer were analysed retrospectively. Preoperative US was performed with convex and high-frequency linear transducers and colour Doppler; invasion depth was graded uT1–uT4 according to the integrity of the wall layers and compared with pT of the AJCC 8th edition. Results. US morphological types were distributed as follows: mass-replacing — 19 (55.9%), wall-thickening — 10 (29.4%), intraluminal (polypoid) — 5 (14.7%). Overall agreement between uT and pT was 26/34 (76.5%; κ = 0.68). Exact T-category agreement was 80.8% in the T3–T4 group and 62.5% in the T1–T2 group; binary discrimination between these groups reached 94.1% accuracy. Preservation of the outer hyperechoic line distinguished T2 from T3 with 82.4% sensitivity and 88.2% specificity. Conclusion. US reliably identifies the morphological type of gallbladder cancer and advanced invasion but is limited in discriminating early T categories; the status of the outer hyperechoic line serves as a key staging criterion.
